Exactly how fair will this proposed "fairness" doctrine be?

It will be as fair as the press currently is in Russia. Which means that nay thing that the government doesn't want known is not known. The fairness doctrine(which is a hypocritical name in and of itself) is a liberal method to shut down the opposing voices.
